Our Project
Our Meals to Heal pilot project is scheduled to start this summer (2024) and will see meals provided to up to 30 community members for a six-month period.
Participants will receive 6 evening meals a week and these will be delivered once a week, throughout West Hants, by our team of community volunteers. Meals will be prepared by the trained chefs at the Station Food Hub at Newport Station and will have dietitian oversight to ensure that they are suitable for participants’ medical conditions.
All meals will be provided free of charge, and, in return, we will be asking participants to help us understand if the meals are making a difference, by completing three surveys and participating in a follow up interview. This will let us evaluate the program for the benefits it provides.
We will be asking for referrals from community members who have the following health conditions, which they be finding difficult to manage.
Diabetes
Heart Disease
Cancer Treatment
Frailty Concerns
If you would like to be considered for the program, please download the referral form and ask your medical professional to complete the medical practitioner form. This form can be signed by any medical professional who has access to your medical information and can confirm your medical conditions (this includes your family physician, nurse practitioner, pharmacist, dietitian, social worker, continuing care co-ordinator etc.)
If you do not have a medical professional, please contact us at referrals@mealstoheal.ca
