For Medical Professionals

The Meals to Heal pilot will be starting Medically Tailored Meal deliveries in September 2024. 

We are currently accepting referrals into the program from community members.  If you have patients who are having trouble managing their medical condition or accessing or preparing the food and diets they require, please consider referring them into our program.  If accepted, they will receive meals (at no cost) for a six-month period. 

Our meals are being prepared by trained chefs in a certified local commercial kitchen (The Station Food Hub in Newport Station) and recipes have been assessed by our registered dietitian advisors, for the specific medical conditions.  To help us allocate the appropriate diet, please complete the attached referral form for your patient and return it to us.

Who Can We Accommodate?

For the pilot, we can accommodate those who have primary conditions of diabetes, heart disease or frailty or those who are undergoing cancer treatment.  Please note that we cannot accommodate anyone with renal disease at this time.

As part of our pilot project, we are conducting an evaluation – this will take the form of 3 short surveys and an end of program interview with participants.  As the participant’s medical professional, we plan to contact you at the end of the program to determine if you have noted any benefits associated with the meals.
